installation. This equipment generates, uses and can radiate radio frequency energy and, if

not installed and used in accordance with the instructions, may cause harmful interference

to other instruments in the vicinity. However, there is no guarantee that interference will not

cesses, procedures, experiments or any other use for which the device is not intended or

cleared by the FDA, or in any manner inconsistent with the instructions for use or labeling.

This device and related accessories are not intended for use in combination with any other

medical devices or in high-risk applications.
